COVID-19 Vaccination Requirement Effective August 23, 2021
To promote a safe and healthful workplace for employees, and to promote the health and safety of students and employees working and learning on the District’s campus and facilities, and members of the community, the District requires that all students and employees be fully vaccinated against COVID-19. Unvaccinated employees are at greater risk of contracting and spreading COVID-19 within the workplace and to the college community that depends on college programs and services. All employees must submit proof of vaccination or request an exemption in Workday by August 23, 2021.
Required Documents
Proof of Vaccination Record
Copy of a vaccination card or other form of documentation from a vaccine provider
containing the date(s) vaccine was administered and the name of the vaccine manufacturer.
Please do not submit any additional medical information. Document must be submitted
in Workday prior to the August 23, 2021 deadline.
OR
Religious Exemption
Employees may request a religious accommodation if they have a sincerely held religious
belief, practice or observance that conflicts with the District’s requirement that
all employees be fully vaccinated against COVID-19. To request a religious exemption,
submit a Religious Accommodation Request form. Document must be submitted in Workday by August 9 to allow time for the review and
approval process prior to the August 23 deadline.

Medical Exemption
Employees may request a medical exemption due to a disability or serious medical condition.
To request a medical exemption, submit a Medical Accommodation Request form to be completed by a licensed medical provider requesting an exemption due to a disability
or serious medical condition. Please be advised that you will need to give a copy
of your job/position description to your medical provider with the request form. Job/position
descriptions can be found on the Human Resources webpage. The Medical Accommodation Request form must be submitted in Workday by August 9
to allow time for the review and approval process prior to the August 23 deadline.
